TERMS OF WEBSITE USE

Please read these terms of use carefully before you start to use the site. By using our site, you indicate that you accept these terms of use and that you agree to abide by them. If you do not agree to these terms of use, please refrain from using our site. Vitality Dietitian abides by the ethical code of the British Dietetic Association and the Health and Care Professions Council to ensure your personal information is protected and confidential.  You can read our GDPR Pivacy Policy Here (LINK TO PRIVACY POLICY) and our Disclaimer here (LINK to Disclaimer).  This site is for use in England and Wales.

Service Agreement

The Definition of ‘’services’’ covers all the services advertised by Vitality Dietitian through this website or any other means.  All service times are not equal, please refer to pricing section for further details or contact us for clarification if needed.  We take payment in advance for all services by Bank transfers or cash only with prior arrangement.  There may be some coaching services like membership payment we may take online on our site via e.g. stripe or PayPal.   We require 50% deposit on booking and full payment up to 48 hours prior to the date of service.  We require a minimum of 48 hour (excluding weekends) notice (via text, phone or email) to cancel any appointments made otherwise full treatment fees will be incurred. Offers cannot be issued in conjunction with each other.

Appointments are normally booked back to back throughout the day, please arrive 10 minutes early, ready for your scheduled appointment. If you are late then we will use the remaining time of your appointment to examine your case.Vitality Dietitian is a subdivision of Sumeya Jakhura Ltd.  Vitality Dietitian reserves the right to refuse or cancel treatment bookings.  All Services are subject to availability. Sumeya may refuse services or stop services in progress if she felt uncomfortable in any way.  Note: We can offer Face to Face Services to most people however we have a narrow entrance and a step, which could be a problem for some people, we can offer virtual consultation instead in such instances.  We cannot guarantee that all services listed or advertised may be available at all times and we reserves the right to withdraw or amend any services at any given time without prior notice.

Vitality Dietitian reserves the right to change price/ discount/ loyalty schemes without any notice.  Whilst the Dietitian will ensure professional practice in consulting & providing relevant advice and resources, the client takes full responsibility for following/ maintaining the advice given and arranging any follow up required.  It is the client’s responsibility to ensure relevant other healthcare professionals are sought and followed up where suggested by Vitality Dietitian.  Vitality Dietitian does offer a referral letter service (there may be additional cost to this).  There may be additional service charges for GP letters, reports and bespoke plans.  Identity verification may be necessary to obtain discounts or when using Health Insurance.

CONSULATION AGREEMENT

We request all consultation forms are returned by email, a minimum of 3 working days before the consultation. It is important you complete the questionnaire, if it is partially filled, it is likely that you will not gain the most out of your consultation and extra time may need to be booked in to provide you with comprehensive advice.

Email correspondence is not a substitute for face-to-face, Skype or telephone contact. It is fine if it is used as a means to clarify something in relation to your programme, however, more lengthy email or text communication will be charged in line with our hourly rates.

It is the client’s responsibility inform and update Vitality dietitian if, at any point during their programme, they change their personal information including contact details or have any medical changes, having an operation or any type of surgery.

You must follow the advice diligently and commit to it fully to gain results as advice alone is not sufficient, it relies on the client’s commitment and actions they take.  Most change in diet and lifestyle can produce gradual effects on energy levels, health, blood parameters and health conditions and are not necessarily instantaneous.

REFUND POLICY/ DEPOSIT/ CANCELLATION POLICY

There are No Refunds except in exceptional circumstances that are out of your control. This is at the discretion of Sumeya Jakhura of Vitality Dietitian.
All sales are final, and no refunds are available. The effectiveness of our advice and plans are mainly reliant by you and your commitment.

Vitality Dietitian is committed to deliver the best service possible.  Should you feel you want a refund please email info@viatlitydietitian with a valid reason, we do not guarantee a refund.If you feel you selected and paid for the incorrect service and wish to change the service selected, please email us at info@vitalitydietitian.co.uk.

Vitality Dietitian shares online booking system with The Amber Clinic.

With much thought and consideration and due to last minute cancelations and no shows, we have updated our cancellation policy (for Vitality Dietitian & The Amber Clinic) as follows from January 2022:

We require a minimum of 48-hour notice to cancel any appointments made otherwise full treatment fees will be incurred.  Cancellations or amendments including cancelling part of a Service of treatment with less than 48 hours notice or no shows will result in loss of your booking fee deposit.  We require a £15 non-refundable deposit for all treatments booked at The Amber Clinic, where a service price is lower than the fixed deposit, the lower value will be applied. The deposit will be used as part payment towards your service.  The remainder of balance will be paid on attendance of appointment. For group bookings, longer bookings (2+ hours) and if you have previously failed to adhere to cancelation policy, a minimum of 50% deposit on booking, will be required and the full balance to be paid 7 days prior to the appointment.  If you fail to attend twice for your scheduled appointments we reserve the right to refuse future appointments.

1) Choose your booking wisely. We will ask you for a £15 deposit (or where a service price is lower than the fixed deposit, the lower value will be applied) to secure your booking.

2) Look out for a confirmation email or text, it is a courtesy only.  However,  you do not receive one, please attend your scheduled appointment or contact us to double check. 

3) We do understand that things can come up, so don’t panic! Just give us at least 48 hour’s notice before the time of the appointment and we can reschedule your appointment.  Vitality Dietitian shares online booking system with The Amber Clinic.  You can reschedule your Vitality Dietitian bookings on https://theamberclinic.book.app  yourself, or email us on info@theamberclinic.com or call/ text us on 07984780372.  If you give us less than 48 hours notice or no show for your slot, we will still try our best to re book your service/ treatment(s) but we reserve the right to retain your full deposit to recover our cost. 

4) Whereby we are unable to honour your appointment due to unforeseen circumstances you will be notified at the earliest convenience on the contact information you have provided us with.  We will endeavour to re arrange your appointment, you may be offered an alternative therapist/ professional, date or time.  Any deposits taken or online in this instance will not be lost. 

5) We do not enjoy retaining deposits, we would much prefer to give our clients full value of their booked treatment and deliver a result!  Just as you make a commitment to your appointments with us, likewise we are also making a commitment to you to be ready with the necessary staff, room, equipment and resources. We also deny other customers who may have desired the same slot, so that we are ready to treat you upon your arrival. We promise to serve you with quality from our skilled therapists/ professionals and deliver results through our cutting edge technology.  We are committed to you!  

 

MOBILE SERVICES

Occasionally when mobile Services e.g. Company Visits or Home Visits are offered there is a standard charge of £5 within 5 miles radius.  Beyond that is 40p per mile extra.  Face to Face available in Leicester only.  Maximum Travel is Midlands for corporate work.

NUTRITION INTERVENTION

Please note Sumeya Jakhura is a UK Registered Dietitian practicing from the United Kingdom.What to Expect from me (Sumeya Jakhura RD) as a Health Care Professional and what I Sumeya Jakhura RD will expect from you as a client:I Sumeya Jakhura RD am Registered with the Health Professions Care Council (HCPC) and abide by their standards of proficiency. I Sumeya Jakhura RD will give you up to date evidence based guidelines in line with British Dietetic Association’s Code of Conduct.I Sumeya Jakhura RD cannot guarantee weight loss with our weight management programmes because there are numerous influencing factors.

I Sumeya Jakhura RD cannot guarantee existing, retrospective or prospective health conditions will be improved, prevented or cured from dietetic intervention.By entering or providing us with your personal your data, you are allowing Vitality Dietitian to access and store your data for the purpose of dealing with your enquiry with a view to providing services to you, in accordance with our data protection, privacy and cookies policy.Upon submitting an enquiry via our contact form, an email will be sent to one of the team to respond. To comply with GDPR, your data will be kept for no longer than one year after your last correspondence. Should you wish to remove your personal data prior to this date please contact sumeya at info@vitalitydietitian.co.uk.

If you instruct us to provide clinical services to you, your data will be stored for eight years in accordance with the BDA and HCPC Standards for Records and Record Keeping.By submitting enquiry and initiating communication in any way you understand this statement and it will be understood that you are happy to proceed.

RELIANCE ON INFORMATION POSTED & DISCLAIMER

The materials contained on our site including Vitality Dietitian (& any nutrition related information on The Amber Clinic site) and any other directly linked media sites such as facebook, instagram, You Tube, Twitter etc. are provided for general information purposes only (for adult population only) and do not claim to be or constitute legal or substitute for seeing a Registered Dietitian or another qualified healthcare professional and shall not be relied upon as such.
